Alec Bohm delivered the decisive blow at Truist Park, smashing a two-out solo home run in the top of the 10th inning to put Philadelphia ahead for the first time since the seventh inning.
The victory sets up a winner-takes-all Game 3 between the National League East rivals on Thursday, with the series winner advancing to face the defending MLB champions, the Los Angeles Dodgers, in the next round.
Philadelphia had trailed 3–1 before Kyle Schwarber and Bryce Harper changed the momentum in the eighth inning.
Schwarber launched a solo home run off Dylan Lee before Harper followed with another homer, bringing the Phillies level at 3–3. The back-to-back blasts were Philadelphia's first postseason pair of consecutive home runs since 2007.
Harper's home run was particularly significant after he had misplayed a fly ball in the seventh inning, allowing Atlanta to take the lead.
Bohm then completed the comeback in the 10th, sending a 357-foot drive over the right-field wall against Braves reliever Didier Fuentes for his third career postseason home run.
Philadelphia's bullpen also held firm in the closing stages, with Andrew Painter producing three scoreless innings in his postseason debut.
Painter faced serious pressure in the bottom of the 10th after Mauricio Dubón and Michael Harris II reached base, but he struck out Dominic Smith to end the game. The final strike was confirmed through Major League Baseball's Automated Ball-Strike Challenge system, making it the first postseason game to end on a successful strike-zone appeal.
The Braves had earlier taken control after Philadelphia opened the scoring in the first inning. Matt Olson helped Atlanta respond before Drake Baldwin's seventh-inning double and Olson's sacrifice fly put the Braves 3–1 ahead.
Despite the defeat, Atlanta had strong opportunities, leaving 11 runners on base during the game.
The series now goes to a decisive third game, with both teams one victory away from the National League Division Series.
• Game 1: Atlanta Braves 5–3 Philadelphia Phillies
• Game 2: Philadelphia Phillies 4–3 Atlanta Braves — after 10 innings
• Series: Philadelphia Phillies 1–1 Atlanta Braves
