Organisers confirmed the seven-time Grand Slam champion’s absence from the ATP Masters 1000 event, which runs from 13-23 August. Alcaraz was the defending champion after winning the title last year.
“We know Carlos is doing everything he can to get back to playing tournaments as soon as possible,” Cincinnati tournament director Bob Moran said in a statement. “We wish him the best with his recovery and look forward to welcoming him back to Cincinnati in the future.”
The 23-year-old world number two has been sidelined since withdrawing from the Barcelona Open in the second round in April. He was unable to defend his French Open title and missed Wimbledon while recovering from the injury.
Alcaraz had signalled he was close to a return by posting training videos in recent weeks and was expected to use Cincinnati as a key warm-up for the US Open. The Winston-Salem Open, which begins on 23 August, remains a possible final opportunity for him to gain match practice before Flushing Meadows, though he has yet to confirm participation in the ATP 250 event.
Alcaraz began the season by winning the Australian Open, becoming the youngest man to complete the career Grand Slam. He followed last year’s Cincinnati success by claiming his second US Open title.
The US Open gets under way on 30 August.
